Day Services Admission Procedure

Admission criteria:

Must be at least sixteen (16) years old
Shawnee County Resident

Admissions application (Printable PDF File)

Admissions information to be included when submitting with your application:

Copy of Health Assessment completed within the last year
Copy of psychiatric evaluation completed within the last 2 years
Copy of birth certificate or Kansas ID
Copy of Social Security Card
Copy of guardianship papers (if applicable)
