Why? Helping Individuals and Families

How? With Funding and Professional Supports

Those Who are Eligible

  • Any individual with a developmental disability (K.S.A. 39-1803).
  • Individuals, five years or older, receiving funding through the Home and Community Based Service's MR/DD Waiver.

Assessment:

  • Learning what is important to individuals and how they want their life to look by talking with them and the people who really know them.
  • Helping people to determine what services they need by arranging for professional evaluations.
  • Helping people to complete referral or placement packets once needed services have been identified.

Development:

  • Helping people to build a network of natural supports, including their circle of support and service providers.
  • Helping the person and his or her support network to develop a person-centered support plan to drive the services received.
  • Ensuring that services are provided in accordance with the person-centered support plan.
  • Empowering persons served to access needed services and entitlements.
  • Advocating for change where barriers exist in the service system.

Referral:

  • Researching available services.
  • Developing natural supports.
  • Helping people to access integrated services and supports, including recreation.
  • Linking identified supports and services that best ensure the attainment of the person's preferred lifestyle.
  • Providing information about services and resources.
  • Making referrals for needed services.
  • Linking the person to community resources, supports, and services.
  • Evaluating the person's satisfaction with supports and services—making adjustments to the plan as necessary.

Monitoring and follow up:

  • Determine if services are being delivered and the person is satisfied with the services.
  • Ensuring necessary follow up on services is completed.
