Admission Procedure

Admission criteria:

• Must be at least sixteen (16) years old
• Shawnee County Resident

Admissions application

Admissions information to be included when submitting with your application:

• Copy of Health Assessment completed within the last year
• Copy of psychiatric evaluation completed within the last 2 years
• Copy of birth certificate or Kansas ID
• Copy of Social Security Card
• Copy of guardianship papers (if applicable)
